Where's the bug submission form in Win2K

Hi Gang,
I'm not sure if it's just for Internet Explorer or if it was for the OS as well, but I do recall coming across a bug feedback form in sort part of Windows or IE. My bug is related to Task Manger in Win2K Sp3 so if the form is only for IE then that would suck. None the less does anyone know what I'm talking about? Where to find it? and if it's just for IE only?
